This document is an example of the required WEEKLY WAGE and HOUR report that can be adopted for use by local systems. Place your school name on the September schedule. Review the entire document/form to become familiar with the items. Record the pay rate/hourly wage. The document will auto sum at the end of each reporting period after recording the hours completed weekly.
